Designed for individuals with one to two chronic health conditions. This plan includes all the benefits from Acute Care plus the following:
- Biannual visits
- Biannual lab work
- Quarterly HM
- Coordination of age-appropriate screening measures
- Referral to appropriate specialists as needed
- Case manager
Advanced Primary Care
Designed for individuals with multiple chronic health conditions. This plan includes all the benefits from Acute Care plus the following:
- Quarterly visits
- Quarterly lab work
- Coordination of age appropriate screening measures
- Coordination of care with specialists

Intensive Care
Designed for individual with declining health requiring frequent visits, home care, hospice, and frequent hospitalizations. This plan includes all the benefits from Acute Care plus the following:
- 4-6 week visits

- Coordination and monitoring of home care
